Exhaled Nitric Oxide and its role in the treatment of Asthma – Part I
In a series of discussions substantiated by findings and excerpts from reputed medical journals and scientists, we strive to understand the importance of exhaled nitric oxide and its role in the treatment of asthma.
In the first part of the series, we discuss the findings from the reputed New England Journal of Medicine on Use of Exhaled Nitric Oxide Measurements to Guide Treatment in Chronic Asthma.
In medicine, exhaled nitric oxide (eNO) can be measured in a breath test for asthma which is characterized by airway inflammation. Nitric oxide (NO) is a gaseous molecule produced by certain cell types in an inflammatory response. Exhaled NO (also referred to as FENO) is a promising biomarker as a guide to therapy in adults and children with asthma. The breath test has recently become available in many well-equipped hospitals in developed countries. Clinical trials have looked at whether tailoring asthma therapy based on eNO values is better than conventional care, in which therapy is gauged by symptoms and the results of lung function tests.
An excerpt from the New England Journal of Medicine in a study by Andrew D. Smith, M.B., Ch.B., Jan O. Cowan, Karen P. Brassett, G. Peter Herbison, M.Sc., and D. Robin Taylor, M.D. details the trial to establish the use of eNO in the measurements to guide treatment in chronic asthma.
The findings of the study are detailed below.
Background
International guidelines for the treatment of asthma recommend adjusting the dose of inhaled corticosteroids on the basis of symptoms, bronchodilator requirements, and the results of pulmonary-function tests. Measurements of the fraction of exhaled nitric oxide (FeNO) constitute a noninvasive marker that may be a useful alternative for the adjustment of inhaled-corticosteroid treatment.
Methods
In a single-blind, placebo-controlled trial, the investigators randomly assigned 97 patients with asthma who had been regularly receiving treatment with inhaled corticosteroids to have their corticosteroid dose adjusted, in a stepwise fashion, on the basis of either FeNO measurements or an algorithm based on conventional guidelines. After the optimal dose was determined (phase 1), patients were followed up for 12 months (phase 2). The primary outcome was the frequency of exacerbations of asthma; the secondary outcome was the mean daily dose of inhaled corticosteroid.
Results
Forty-six patients in the FeNO group and 48 in the group whose asthma was treated according to conventional guidelines (the control group) completed the study. The final mean daily doses of fluticasone, the inhaled corticosteroid that was used, were 370 ?g per day for the FeNO group (95 percent confidence interval, 263 to 477) and 641 ?g per day for the control group (95 percent confidence interval, 526 to 756; P=0.003), a difference of 270 ?g per day (95 percent confidence interval, 112 to 430). The rates of exacerbation were 0.49 episode per patient per year in the FeNO group (95 percent confidence interval, 0.20 to 0.78) and 0.90 in the control group (95 percent confidence interval, 0.31 to 1.49), representing a non-significant reduction of 45.6 percent (95 percent confidence interval for mean difference, ¡78.6 percent to 54.5 percent) in the FeNO group. There were no significant differences in other markers of asthma control, use of oral prednisone, pulmonary function, or levels of airway inflammation (sputum eosinophils).
Conclusions
With the use of FeNO measurements, maintenance doses of inhaled corticosteroids may be significantly reduced without compromising asthma control.

Exhaled Nitric Oxide and its role in the treatment of Asthma Part III
In the third part of the ongoing series, we discuss the findings from the reputed Thorax – An International Journal Of Respiratory Medicine.
An excerpt from Thorax – An International Journal Of Respiratory Medicine in a study by A Jatakanon, S Lim, S A Kharitonov, K F Chung, P J Barnes details the Correlation between exhaled nitric oxide, sputum eosinophils, and methacholine responsiveness in patients with mild asthma.
Background
Eosinophils in induced sputum and exhaled nitric oxide (NO) are currently used as non-invasive markers in the assessment of airway inflammation in asthma. As both sputum eosinophils (%) and exhaled NO are raised in asthmatic subjects not receiving inhaled steroids and decreased following corticosteroid therapy, a relationship between them is plausible.
Methods
Exhaled NO was measured by chemiluminescence analyser, sputum induction by 3.5% saline inhalation, and bronchial responsiveness was measured as PC20FEV1 methacholine in 35 stable asthmatic patients using beta 2 agonist alone and the correlation between these non-invasive markers of airway inflammation was studied.
Results
There were significant correlations between exhaled NO and PC20 (r = -0.64), exhaled NO and sputum eosinophils (%) (r =0.48), and also between sputum eosinophils (%) and PC20 (r = -0.40).
Conclusion
The correlation between exhaled NO and PC20 suggests that exhaled NO or the mechanisms leading to its increase may contribute to airway hyperresponsiveness in asthma. Furthermore, the relationship between sputum eosinophils (%), exhaled NO, and PC20 highlight the potential use of eosinophils (%) in induced sputum and exhaled NO to monitor the severity of asthma.

Selective Catalytic Reduction – How NOx Reduction Make the World Go Green
With the turn of the century, there has been an increase in concern over the health of the planet. Scientist and the government work hard to try and make changes to the environment. There was a realization that without the world, there was no life and therefore, it was imperative to save the world. With nations continuing to industrialize, it has become increasingly difficult to curb the greenhouse gases and dangerous toxins that are destructive to the world.
One of those toxins is called NOx. NO is nitric oxide and NO2 is nitrogen dioxide. Both of these are components of NOx in different concentrations. Typically, people have no idea that there is NOx around because not only is it colorless, it also has no odor. Because of this, it can unwittingly be released into the environment.
There are a plethora of places that have nitrogen oxide emissions. Almost half of all these emissions come directly from automobiles. At the turn of the century, a lot of people were driving around in large, gas guzzling SUVs that did nothing more than produce an increase in these emissions. Ironically, the economic downturn has shaped the society into a more green-friendly nation. However, anything that uses some sort of a hydrocarbon for its gas will produce some form of NOx. Typically, most are found to be NO.
Often times, people question why this matters. They don’t appreciate the environmental concerns connected to NOx. The most important concern is the effect NOx has on the human respiratory system. NO combines with ammonia which leads to the formation of nitric acid which is an incredibly strong acid that can do a serious amount of damage to the body. Small particles get into the lungs and puncture holes in them which can lead to a potentially fatal result.
There are things that are being done to try and prevent this NOx from becoming too much of an issue. In some areas, one of the big tools being used is selective catalytic reduction. What this does in non-technical terms is create elemental nitrogen and water. Neither of these is harmful on the environment. In more technical terms, urea is used in the engine and then broken down into ammonia. The products of this reaction, after it has gone through a catalyst are those elemental nitrogen and water.
The SCR Catalyst has been shown to do a significant amount of work on the NOx reduction. However, there are other routes, as well, that can be used to try and reduce the NOx concentrations in the environment. One of these is the use of engine management which, as the name suggests, is the control of the amount of emissions in an engine. There are fewer emissions released, but then the engine runs less effectively which results in the build-up of soot.
One final method is a NOx trap which catches the NOx and then catalytically converts that stored NOx, through acts of reduction, into basic N2 and CO2. Nitrogen in its elemental form is not damaging to the environment. While CO2 can be damaging to the environment, it is not nearly as damaging as the NOx gases that are released by the automobiles. CO2 is the lesser of two evils.
What these three techniques have in common is that they all are trying to reduce the NOx levels. However, SCR catalyst is the one that has been shown to have the most effect on the reduction of NOx concentration levels. There is no concern of a lower effectiveness of the automobile and there is no need for a “lesser of two evils” approach because most of the NOx is converted into water and elemental nitrogen. The only downfall is that there needs to be some way of getting the urea into the system. If that occurs, this is definitely an effective tool.
Each technique has its ups and downs. Some work better than others (SCR), but even that has its negatives. It is important to determine what the cost to productiveness ratio is that automobile companies are willing to spend. What is too much? What is clear, though, is that the environment is not getting any stronger and if people are truly concerned about it, they need to take the necessary steps to prevent against further degradation, if that means spending a little more money, that might be the necessary course of action.
